Alcohol-Related Liver Disease and Dynamic Liver Stiffness

Alcohol withdrawal provided a natural model showing that a substantial component of liver stiffness can fall within days as inflammatory injury resolves.

In patients with alcohol-related liver disease, liver stiffness frequently decreases during detoxification. This rapid change cannot be explained by regression of fibrosis and instead reflects resolution of steatohepatitis and related tissue-pressure changes. The effect is clinically important because measurements obtained during active injury may overestimate fibrosis stage.

Subsequent work showed that the PNPLA3 I148M variant is associated with more pronounced inflammatory injury and delayed resolution of stiffness after alcohol withdrawal, linking genotype, disease activity and dynamic elastography.
